We are a locally based home care agency seeking special health care clinicians to join our administrative team! Are you looking to transition from direct patient care, but still want to help people? The Work:
You'll provide communication to clinics, hospitals, patients, and their caregivers, to deliver a seamless care process. You'll support field clinicians with medication verification, plan of care coordination, obtain verbal orders for services needed, and provide other support as requested. You'll collaborate to provide the highest quality of care for our patients. This work is fully in-person in our office in Little Canada.

The Pay:
$64,480 - $66,000 exempt, annual salary.
Job Duties:
Process referrals as assigned.
Accurately build patient medical record.
Schedule patients as assigned.
Prepare medication profiles prior to admission visits.
Make calls to physician offices to obtain verbal orders.
Receive verbal orders and update patient chart accordingly.
Order DME/supplies as requested.
Shadow and reverse-shadow new team members for training purposes.
Successfully complete patient satisfaction calls.
Assist in on-call coverage (extra pay) during non-office hours.

Prolonged periods of sitting at a desk and working on a computer
Job Type: Full-time, Monday-Friday 8:30AM - 5:00PM
Experience:
2 years of medical office experience and/or direct patient care.
Ability to provide excellent customer service
Ability to multi-task in a fast-paced environment.
Ability to work and communicate professionally with patients, medical partners, employees, and supervisors.
Ability to work independently and as part of a team.
Required:
Possess and maintain current Minnesota Occupational Therapist Assistant certification or Physical Therapist Assistant license in good standing required.
Knowledge of medical terminology.
Excellent communication skills.
Good judgment and decision-making skills.
Strong attention to detail.
Technologically proficient and driven by accuracy.
Act as a role model of Advanced Medical Home Care’s values and mission.
Working Condition:
Ability to sit, stand, type, see, hear, and communicate verbally, as well as in writing.
Ability to complete job tasks within 40 hours per week as stated in offer.
